Clinical features of hypotension in continuous ambulatory peritoneal dialysis patients and related nursing management

Abstract: Objective To investigate the clinical features and influencing factors of hypotension in continuous ambulatory peritoneal dialysis patients, and to summarize related nursing measures. Methods 〓A retrospective study on clinical features of hypotension in continuous ambulatory peritoneal dialysis patients was conducted. Patients were divided into short-tem group(< 36 months of dialysis treatment)and long-term group(≥36 months of dialysis treatment)according to the duration between onset of dialysis and occurrence of hypotension. Clinical features of hypotension and related influencing factors were analyzed. Results 〓Totally 157 patients were included in the study. The main cause of hypotension was cardiac insufficiency in 37(23.57%)cases, followed by ultrafiltration volume excess in 33(21.02%)cases, insufficient blood volume in 21(13.38%)cases, improper use of anti-hypertensive drugs in 20(12.74%)cases. Of 157 patients, transient hypotension was found in 96(61.15%)cases, repeated attack of hypotension was found in 38(24.20%)cases and refractory hypotension was found in 23(14.65%)cases. The proportion of patients with transient hypotensionin the short-tem group(n=77)was higher than that of patients in the long-term group(n=80), with a significant difference(P<0.05). The proportion of patients with repeated attack of hypotension and refractory hypotension in the short-term group was lower than those of patients in the long-term group, respectively(P<0.05). Conclusion 〓The main causes of hypotension were cardiac insufficiency, ultrafiltration volume excess, insufficient blood volume and improper use of anti-hypertensive drugs.Different clinical features of hypotension were found between patients with different duration of dialysis treatment. Targeted nursing measures should be carried out according to the clinical features and potential influencing factors of hypotension.
